SELF-SWAB COVID-19 TEST KIT PICK-UP/DROP-OFF

COVID-19 PCR self swab test kits are offered free of charge for both asymptomatic & symptomatic/close contact individuals. Test kits must be ordered ahead of time (not available on a walk-in basis) and you must meet specific criteria to be eligible for testing (click here to view criteria).

Please follow steps outlined below to order your COVID-19 PCR self swab test kit.

STEP ONE

You must have an agent who is available to pick up the kit on your behalf. The agent must be asymptomatic, not a household contact of a COVID-19 positive individual or have tested positive on a rapid antigen test.

You must be comfortable with completing a self swab. The pharmacy staff will NOT be able to assist you in collection of the swab. The process is simple and instructions are provided in the kit.

If you do not have an agent available to pick up on your behalf, or are not comfortable collecting a self-swab, please request an in-store COVID-19 test

STEP TWO

Complete the COVID-19 PCR test kit request/screening form

Please double check all information before submitting the form. The information that you provide will be used to assess your eligibility & prepare lab requisition form. In addition, the information will be used by the lab to upload your results on the e-health portal. If there is any discrepancies between information you entered and what is on your health card, you will not be able to access your results online.

You will receive a confirmation e-mail once the test kit is ready to be picked up. Our goal is to get each kit ready within 2 hours, but it can take up to 24 hours. Please do not call the store to inquire about your test kit.

STEP THREE

Send an agent on your behalf to pick up COVID-19 PCR self swab test kit. They can be picked up 9AM TO 9PM AT THE PHARMACY DROP-OFF SECTION

STEP FOUR

Collect your sample by following the instructions provided in the test kit. Do not remove the requisition form that is included in the self-swab test kit bag.

Return your collected self-swab within 2 hours to the pharmacy

STEP FIVE

After you complete your covid-19 PCR test, your results can be accessed online. Click here to access your results. Results take typically 48 hours to arrive but currently can take up to 6 days .

Please do not contact the store unless 6 days have passed from your swab drop off time. All inquiries related to PCR test results must be e-mailed. Not all pharmacy staff are involved in PCR COVID testing or will be able to answer your questions. If you have not received your result after 6 days, e-mail us at hollandstreetpharmacy@gmail.com

We will contact you with your results if you meet one of the below categories:

You have a positive result/cancelled test

You do not have a OHIP card

You have a red/white OHIP card [please let us know by e-mail (hollandstreetpharmacy@gmail.com)]

Frequently Asked Questions:

a. I do not have an agent to pick up the kit on my behalf. Am I able to come pick it up myself ?

At this point, the answer is No. We are currently designing our process to have pick up of kits by asymptomatic/non-exposed individuals.

b. My results are not on the e-health Ontario portal? What should I do now?

Results can take up to 6 days to arrive & update on the e-health portal. Due to increased demand on testing, it is taking almost 6 days for results to arrive. If it has been more than 6 days since your test kit drop off time and you have not received your result, please email us at hollandstreetpharmacy@gmail.com and we will call the lab on your behalf.

Keep in mind we do not process the samples or upload them on the e-health portal. Our role is to collect the sample and send it to the lab. Currently, the lab is backed up and most result take 6 days to get updated on the portal. We do not have control over when the portal is updated with results. If your results have not arrived 6 days from your PCR test kit drop off date, email us so that we can contact the testing lab on your behalf.
